1.) What is the mission of My Health Briefcase?
We put your health records onto the computer with Internet Medical History
storage and patient assistant services, which are secure. This is especially
important for seniors, travelers, children at camp and daycare and those
with disabilities .It is our mission to take the pain out of paperwork and
make medical history information more convenient for everyone.

2.) How do I begin my record?
Enter your information by clicking on Register Tab on the top of our Home
page. Your user name and password, which are selected specifically for you
will be emailed back to the address you provide. When you receive this,
you may proceed to the next step.

3.) What is the next step?
You will be asked to complete important health information about yourself
at this time so please be prepared to do so, allow at least 10 minutes.
When your registration information is complete, you will need to click the
save button before advancing to the next screen.

4.) How much detail should I go into?
Be sure to include as much detail as possible on all your answers. Always
enter the most complete information possible for every question. In the
section on Family History try to include information on family members who
have illnesses or who may have perished due to illness. There is plenty
additional space in every section to go into detail.

5.) How do I complete the section on childhood immunizations?
The section on Childhood Immunizations is very important to parents and
children. You will find it very useful to keep this section up to date.
Everyone will enjoy the easy access to this information over the years,
especially when these records are needed for camp, school or changing addresses,
Insurance or Doctors. To make the record official all that is needed is
a Doctor's signature. You can easily and at any time print this section
on paper to place in your child's record at the Doctor's office or to keep
at home. As a convenience, we have provided you with a schedule of the latest
recommendations for childhood vaccinations in this section. Please talk
to your child's Doctor if immunizations listed have not been given to your
child.

7.) How do I view my record?
When you have finished entering your information, please take the time to
view it by clicking on "View Medical History" on the right upper
corner of the data entry page.

8.) How do I print my wallet card?
If the information you have entered is correct and you are satisfied with
it, please be sure to click on "Print Wallet Card" on the right
upper corner of the data entry form. You can then print your Wallet/Purse
Identification Card using the "print" command from your browser.
Please use your printer's normal 8.5x11" paper and cut the card out.
This card should be stapled or taped to the back of your Medical insurance
card so that it is readily available to you and in case of an emergency
to medical personnel treating you.

9.) Tell me about my password.
Please do not give your password or ID card to anyone. You may give your
password to your Physician for review of your information if you choose,
but please do not share your password with anyone else to avoid compromising
the security of your information. Obviously, in an emergency, you will ask
that the Emergency treating Physician go on the Internet using the information
on your card to access your files. In rare circumstances, a request for
information may result from you being unable to communicate. In such cases,
your wallet card will be used to access your records, by agreeing to the
terms and conditio0ns of this site, you have given permission for your records
to be accessed in emergency situations.

10.) How does my Doctor share this information?
When you give your doctor your permission to enter the site and your password,
special areas of the site are intended for Medical personnel to record findings
of examinations and tests. And for medical billing purposes.
